Professional credentials
Entitled Document Services is the trade name of Entitled Document Services, LLC, an Arizona limited liability company (ACC Business ID 25108078). Document preparation services are provided by Sammijo Johnson, an Arizona Certified Legal Document Preparer certified and regulated by the Arizona Supreme Court under Arizona Code of Judicial Administration section 7-208.
Not an attorney
Sammijo Johnson is not an attorney and is not employed by or affiliated with a lawyer or law firm. Communications with a legal document preparer are not protected by the attorney-client privilege. She does not represent that she is an attorney, hold herself out as an attorney, or provide legal services outside the scope of her Certified Legal Document Preparer credential. For matters that require legal advice or courtroom representation, clients are referred to a licensed Arizona attorney.
No attorney-client relationship
Use of this website does not create a professional-client relationship between you and Entitled Document Services, LLC. Submitting a contact form, sending an email, or requesting a service does not, by itself, create an engagement. A professional relationship begins only through a signed written agreement that describes the services and the fee.
Communications with a legal document preparer are not protected by the attorney-client privilege. Information you share may be subject to disclosure if ordered by a court. We treat your information with discretion as a professional matter, but it does not carry the legal protections that apply to communications with an attorney.

We are not attorneys and we are not accredited representatives of a recognized organization. We do not select forms, assess eligibility, advise on immigration options or consequences, interpret form questions, or represent you before USCIS, the immigration court, or any agency. Only a licensed attorney or a United States Department of Justice accredited representative may give immigration legal advice or represent you.
No guarantee of results
Every matter is unique. Outcomes depend on the facts, applicable law, the conduct of other parties, judicial discretion, and other factors outside the control of any document preparer. Nothing on this website guarantees or predicts a particular result.
Fees and costs
Preparation fees shown on this website do not include court filing fees, recording fees, publication costs, process server fees, parenting class fees, or government filing fees such as USCIS fees. Those are set by the court or agency and are paid separately. Our rates and charges are disclosed and itemized in writing before any work begins, and current court and agency amounts are published on our Court and agency fees page.
What a certified legal document preparer may and may not do
Under Arizona Code of Judicial Administration § 7-208, a certified legal document preparer may prepare legal documents for people who are not represented by an attorney, provide general legal and factual information, make forms available, and file, record, and arrange service of documents. A preparer may not give specific legal advice, opinions, or strategy; may not represent anyone in court or before an agency; may not attend court to assist a party unless permitted by law; and, with narrow statutory exceptions, may not sign the documents she prepares. Every document we prepare carries the preparer's name, the title Arizona Certified Legal Document Preparer, certificate number AZCLDP No. 82291, and the business entity name, as the rules require.
Competence and timeliness
We accept only assignments we are competent to complete accurately and on time, and we decline those we are not. We make a good-faith effort to meet promised delivery dates and notify you promptly of any delay. We do not charge contingent fees, accept anything of value tied to the outcome of a matter, or pay referral commissions.
Complaints
Certified legal document preparers are regulated by the Arizona Supreme Court. Complaints may be directed to the Certification and Licensing Division, Arizona Supreme Court, 1501 W. Washington St., Suite 104, Phoenix, AZ 85007, or through azcourts.gov/cld. Nothing in our agreements or on this website limits that right.
