No. We cannot tell you which document to file, whether you qualify, or what strategy to use. Those questions call for a licensed attorney, and we are glad to suggest where to find one. What we do is prepare your chosen documents accurately, completely, and on time.

Every document is a flat fee, published on this site, quoted in writing before you commit. Court filing fees and USCIS fees are set by the government, listed separately, and paid to the agency.

Our service is fully bilingual: we work with you in Spanish, prepare your documents in English as required (you file them, or add our filing service), and certified English translations of Spanish-language supporting documents are available.

Chapter 7 petition typing by a bankruptcy petition preparer under 11 U.S.C. § 110: we type the petition, schedules, and statements from the information you provide. We cannot advise whether to file, which chapter to choose, which debts are dischargeable, or how to protect property, and we do not appear at the meeting of creditors. Meet Sammijo Johnson, MLS, M.S.
Sammijo Johnson, MLS, M.S., is the founder of Entitled Document Services, LLC and an Arizona Certified Legal Document Preparer, AZCLDP No. 82291. She built the company on a simple premise: that meaningful help with legal paperwork should be accessible, transparent, and deeply attentive to the individual circumstances of every client.
Sammijo has called Arizona home for more than fifteen years. She has been happily married for more than six years and is the mother of three little girls, ages three, five, and seven, and she is a three-time Sun Devil, having earned three degrees from Arizona State University. Her work is grounded in the belief that the court and immigration systems are hard enough to navigate without the paperwork being the part that defeats you.
Her preparation for this work has been deliberate. She holds a Master of Legal Studies from the Sandra Day O'Connor College of Law at Arizona State University, with training in legal analysis, contracts, legal writing, and research, and she is pursuing a Doctor of Law and Policy at Northeastern University. Her doctoral research examines access to justice and why so many families in civil and family court end up facing the system without help at all. That research is the reason this company exists.
She prepares every document with precision and genuine care, and she is always clear about what a document preparer can and cannot do. When a matter calls for legal advice or courtroom representation, she says so and points clients toward a licensed attorney.

What a certified legal document preparer may and may not do
Under Arizona Code of Judicial Administration § 7-208, a certified legal document preparer may prepare legal documents for people who are not represented by an attorney, provide general legal and factual information, make forms available, and file, record, and arrange service of documents. A preparer may not give specific legal advice, opinions, or strategy; may not represent anyone in court or before an agency; may not attend court to assist a party unless permitted by law; and, with narrow statutory exceptions, may not sign the documents she prepares. Every document we prepare carries the preparer's name, the title Arizona Certified Legal Document Preparer, certificate number AZCLDP No. 82291, and the business entity name, as the rules require.
Competence and timeliness
We accept only assignments we are competent to complete accurately and on time, and we decline those we are not. We make a good-faith effort to meet promised delivery dates and notify you promptly of any delay. We do not charge contingent fees, accept anything of value tied to the outcome of a matter, or pay referral commissions.

Certified legal document preparers are regulated by the Arizona Supreme Court. Complaints may be directed to the Certification and Licensing Division, Arizona Supreme Court, 1501 W. Washington St., Suite 104, Phoenix, AZ 85007, or through azcourts.gov/cld. Nothing in our agreements or on this website limits that right.

When you use our contact or request form, we collect the information you provide, which may include your name, email address, phone number, county, and a description of your matter. To prepare your documents, we collect the additional information you give us during intake, which may include personal, family, financial, or property details necessary for the documents you select.
How we use your information
We use your information to respond to your inquiry, to prepare the documents you request, to communicate with you about your matter, and to process payment. We do not sell your information, and we do not share it for advertising.
Service providers
Confidentiality
We treat the information and documents you provide as confidential and use them only to prepare your documents and administer your matter, subject to disclosure required by law. As Arizona rules require us to state, communications with a legal document preparer are not protected by the attorney-client privilege and may be subject to disclosure if ordered by a court.
Signed agreements, payments, and messages
When you sign an agreement on this site, we store the agreement text, your typed and drawn signatures, and the signing timestamp with your submission, and a copy is emailed to you. Payments are processed by Stripe on Stripe's secure pages; we never see or store your full card number. Messages sent by WhatsApp are handled under WhatsApp's own privacy terms; please do not send Social Security numbers or full account numbers by chat, and use our secure upload page for sensitive documents.

Entitled Document Services, LLC is not a law firm and does not provide legal advice. Using this website or contacting us does not create a professional-client relationship. A relationship begins only through a signed written agreement describing the scope of services and the fee.
Your responsibilities
You are responsible for the accuracy of the information you provide and for the decisions in your matter, including which documents to file and how to answer the questions on your forms. We prepare documents based on the information and decisions you give us.
Fees and payment
Preparation fees are flat fees, published on our services menu and disclosed and itemized in writing before work begins. Court filing fees, recording fees, publication costs, process server fees, and government filing fees such as USCIS fees are set by the court or agency, are never part of our fee, and are paid by you directly to that court or agency. Current amounts and official links are listed on our Court and agency fees page. Work begins after your agreement is signed and payment or any required deposit is received.
Filing service and advanced court fees
If you add our filing or recording service, we file or record your documents with the court, recorder, or agency on your behalf for the filing service fee shown at checkout, paid by card with your other fees. The court or agency fee itself is passed through at cost, without markup, and is paid only by ACH bank transfer through the secure Stripe payment page linked from our website, never by card, under our separate Court and Agency Fee Payment Agreement. Stripe emails a receipt when the transfer is initiated; we confirm in writing when it clears, hold those funds separately, remit them at filing, and provide the court’s receipt. A court fee is refundable in full if you cancel before we pay the court and is not refundable by us once paid.
Rush preparation
Rush preparation is an optional add-on offered at checkout only on services whose turnaround we control. It is priced by tier and disclosed before payment. Rush work begins immediately upon payment and is non-refundable. Court, agency, and USCIS processing times are set by those bodies and cannot be rushed by us.
Cancellations and refunds
You may cancel for a full refund by written notice to us by email or WhatsApp message within 24 hours after your payment is received, or within 48 hours for orders that include more than one service. The window is measured from the time we receive your payment. After it closes, fees are non-refundable. Rush preparation begins immediately upon payment and cannot be cancelled or refunded. Court, recorder, and agency fees are never refundable once submitted to the court or agency. If a court clerk rejects a document because of a preparation or formatting error we made, we correct and re-prepare it at no charge; that correction is your remedy for preparation errors.
Billing questions, chargebacks, and disputes
If you have a question about any charge, contact us first in writing at billing@entitleddocs.com or by WhatsApp and allow one business day for a response; most billing questions are resolved the same day. By paying, you acknowledge that you are the authorized cardholder, that our services are delivered electronically to the email address you provide, that the refund terms were disclosed to you before payment, and that you will not initiate a chargeback for services delivered as described. A chargeback filed contrary to these terms is a breach of your agreement and may be pursued as a civil claim for the amount reversed, our dispute fees, and our costs and reasonable attorney fees where authorized by A.R.S. § 12-341.01, in small claims court or another court of competent jurisdiction. Before filing any claim, dispute, or chargeback, you agree to give us written notice and five business days to correct the concern. Disputes we cannot resolve informally are resolved by binding individual arbitration under the American Arbitration Association Consumer Arbitration Rules in Maricopa County, Arizona, except that either party may bring an individual claim in small claims court. Nothing in these terms limits your right to file a complaint with the Certification and Licensing Division of the Arizona Supreme Court.
Confidentiality and conflicts of interest
We treat the information you provide as confidential and use it only to prepare your documents, subject to disclosure required by law. Communications with a legal document preparer are not protected by the attorney-client privilege. If an actual or apparent conflict of interest arises in your matter, we will disclose it to you in writing and will not provide or continue services unless you waive it in a writing signed by both parties.
Electronic signatures and records
Agreements are signed electronically. Your electronic signature has the same effect as a handwritten signature under the Arizona Electronic Transactions Act (A.R.S. § 44-7001 et seq.) and the federal ESIGN Act. A complete copy of each signed agreement, including the agreement text, your typed and drawn signatures, and the signing timestamp, is kept on file and provided to you.
Intellectual property
The content, design, and branding on this website are the property of Entitled Document Services, LLC and may not be copied or reused without permission.
Limitation of liability
To the extent permitted by law, Entitled Document Services, LLC is not liable for outcomes that depend on the facts, applicable law, the conduct of other parties, or judicial discretion. Our responsibility is to prepare the documents you select accurately and in accordance with the information you provide.
Governing law
These terms are governed by the laws of the State of Arizona.

1. Acknowledge
Enter the court or agency, the documents, and the exact fee we quoted, then sign the short agreement below with your typed name.
2. Pay now by bank transfer
The Pay now button opens Stripe’s secure page, where you connect your bank account. ACH only, at exact cost, with no card fees and no markup. Stripe emails your receipt right away.
3. We file and send the receipt
ACH clears in two to five business days. We confirm in writing when it does, file or record your documents, and email you the court’s or recorder’s receipt.
Process serving
Two separate charges. Entitled Document Services, LLC charges only its flat preparation fee. Court filing fees, recording fees, publication costs, process-server fees, and government or USCIS fees are set by the court or agency, are not included in our fee, and are paid by you directly to that court or agency. We do not mark up agency fees. If you add our filing service, the court or agency fee is passed through at cost and paid only by ACH bank transfer through Stripe, under our Court and Agency Fee Payment Agreement..

Invoice payments are for services under your signed agreement, and its payment terms apply: you may cancel by email or WhatsApp within 24 hours after payment is received (48 hours for multi-service orders) for a full refund; after that, fees are non-refundable, and rush work cannot be cancelled or refunded; and billing questions come to us first in writing at billing@entitleddocs.com. Disputes are resolved individually by arbitration or in small claims court, as the agreement provides.

Arizona requires a written agreement between you and your document preparer before work begins. Read it, fill in the details from your written quote, and sign below. You will receive a copy by email.
Document Preparation Services Agreement
Parties. This agreement is between Entitled Document Services, LLC ("we," "us") and the client named below ("you"). Document preparation services are provided by Sammijo Johnson, an Arizona Certified Legal Document Preparer, AZCLDP No. 82291, certified and regulated by the Arizona Supreme Court under Arizona Code of Judicial Administration § 7-208.
Not an attorney; no legal advice. Sammijo Johnson is not an attorney, is not employed by or affiliated with a lawyer or law firm, and is not licensed to practice law in Arizona or any other jurisdiction. We prepare the documents you select and may provide general legal and factual information, but we cannot give legal advice, tell you which documents to file, advise you on your rights, options, or strategy, or represent you in court or before any agency. If you need legal advice or representation, you should consult a licensed attorney. Communications with a legal document preparer are not protected by the attorney-client privilege.
Services. We will prepare the documents described below, based on the information and decisions you provide, and deliver them with filing instructions and a completeness checklist. You are responsible for the accuracy of the information you give us and for reviewing every document before it is signed or filed.
Confidentiality. We treat the information and documents you provide as confidential and use them only to prepare your documents, subject to any disclosure required by law. As stated above, communications with a legal document preparer are not protected by the attorney-client privilege.
Fees. Our preparation fee is the flat fee stated below and confirmed in your written quote, which discloses and itemizes our rates and charges in writing before any work begins. Court filing fees, recording fees, publication costs, process-server fees, and government or USCIS fees are set by the agency, are not included, and are paid by you directly to the agency. If a court clerk rejects a document we prepared because of a preparation or formatting error, we correct and re-prepare it at no charge.
Filing service and court or agency fees. We prepare your documents and you file them with our instructions, unless you add our filing or recording service, in which case we file or record your documents with the court, recorder, or agency on your behalf. Our filing service fee is disclosed at checkout and paid by card together with your preparation fee. The court, recorder, or agency fee is different: it belongs to the court or agency, we pass it through at exact cost with no markup, and it is paid only by ACH bank transfer through the secure Stripe payment page linked from our website, never by card, so that card fees are never added to money that is not ours. You receive a Stripe receipt when the transfer is initiated; we confirm in writing when it clears, typically two to five business days, then file or record and send you the court’s or recorder’s receipt. If you cancel in writing before we have paid the court or agency, that fee is refunded in full; once submitted to the court or agency it is not refundable. These terms are set out in full in our Court and Agency Fee Payment Agreement, which you acknowledge when you pay a court fee and which is incorporated into this agreement.
Payment, cancellation, and refunds. Fees are due as stated in your written quote, and work begins once this agreement is signed and payment or any required deposit is received. You may cancel for a full refund by written notice to us by email or WhatsApp message within 24 hours after your payment is received; for orders that include more than one service, that window is 48 hours. The window is measured from the time we receive your payment. After the window closes, fees are non-refundable. Emergency or rush preparation begins immediately upon payment and cannot be cancelled or refunded. Court, recorder, and agency fees are never refundable once submitted to the court or agency. The correction promise above is your remedy for preparation or formatting errors.
Timing. Standard turnaround is stated in your quote. Rush service, when available, is quoted separately.
Billing questions and chargebacks. If you have a question or concern about any charge, you agree to contact us first in writing at billing@entitleddocs.com or by WhatsApp and allow one business day for a response; most billing concerns are resolved the same day. By signing this agreement and submitting payment, you acknowledge and agree that: (1) you are the authorized holder of the payment method used and you authorize the charges described in your written quote; (2) these are custom document preparation services delivered electronically, and delivery to the email address you provide constitutes receipt; (3) the refund policy above was disclosed to you in writing before payment and you accept it; (4) you will not initiate a payment dispute or chargeback for services delivered as described in this agreement, and a chargeback made contrary to this paragraph is a breach of this agreement; and (5) if such a chargeback is filed, we may submit this signed agreement, our delivery records, and our communications with you as evidence, and you remain responsible for the fees for work performed and for any dispute fees our payment processor charges us, to the extent permitted by law. Charges appear on your card statement under our business name, Entitled Document Services.
Dispute resolution. Before filing any claim, dispute, or chargeback, you agree to give us written notice describing the concern and five business days to correct it; the correction promise above is part of that cure opportunity. Except for the small-claims right below, any dispute arising out of this agreement or our services that we cannot resolve informally will be resolved by binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, in Maricopa County, Arizona, or by phone or video, and judgment on the award may be entered in any court with jurisdiction. Either party may instead bring an individual claim in small claims court. A chargeback reversed contrary to this agreement may be pursued as a civil claim for the amount reversed, our dispute fees, and our costs and reasonable attorney fees where authorized by A.R.S. § 12-341.01 or other law, in small claims court or another court of competent jurisdiction. Claims proceed individually and not as class or representative actions, to the extent permitted by law. This agreement is governed by Arizona law. Nothing in this agreement limits your right to file a complaint with the Certification and Licensing Division of the Arizona Supreme Court.
Conflicts of interest. If an actual or apparent conflict of interest arises in your matter, we will disclose it to you in writing and will not provide or continue services unless you choose to waive the conflict in a writing signed by both parties.
Complaints. Certified legal document preparers are regulated by the Arizona Supreme Court. Complaints may be directed to the Board of Legal Document Preparers, Certification and Licensing Division, Arizona Supreme Court, 1501 W. Washington St., Suite 104, Phoenix, AZ 85007, or through azcourts.gov/cld.
Electronic signature. You agree that your electronic signature below has the same effect as a handwritten signature under the Arizona Electronic Transactions Act (A.R.S. § 44-7001 et seq.) and the federal ESIGN Act.
